small update:
Upon slightly further investigation, yes, this does happen in other programs other than Windows Media Player, so I do think that this is a VMS issue, NOT a windows being screwey issue.
I've recreated this using Doom Retro (https://www.doomretro.com), doing the same thing as previous to check if it works with regular synth and VMS, and this has the same effect, by simpily triggering any midi to play (Either a map, text card, or title screen).
I am going to go to sleep, but I'll check back on this in the morning, thanks!

Well, this slowdown only at play start is a really strange behavior 🤔.
Some questions:
- Which are the system specifications? I mean, CPU, RAM, HDD or SSD, ...
- How is the soundfont configured and loaded in VMS? Is the preload feature enabled on VMS on that soundfont? Have you tried to enable/disable preload?
- Do you see any CPU peak when the delay happens?
- Could you please post the MIDI file you're playing?

Your PC is powerful enough to give a good MIDI synthesys.
I've tried to play that MIDI file on my PC with the same soundfont with VanBasco player (my preferred one, never had any issue with it): can't ear any slowdown at the beginning 🤔.
So I've played it with Windows media player and then I've heard a minimal slowdown at start...
Being it very minimal, I've played it a lot of time to exclude it was some kind of self-persuasion and I'm still not convinced it really is.
It surely is much more audible than the one you've sent in your video 🙄.
Could you please test it with VanBasco player?
It's a very lightweight MIDI/Karaoke player, it worths a try.
Please remember to right click on its setup file and run it with Run as administrator option (being it a Win95-era software, it won't ask admin permissions automatically...).
After installation, press ALT+S and select VirtualMIDISynth as output device (no need to configure MIDIMapper).
